About Care Directions
HIV Care Directions(R) (HIVCD), a program of the Area Agency on Aging, Region One, is a nationally accredited, professional case management program that has provided medical and support case management services to HIV positive people 18 and older since 1992.
The HIV Care Directions' case managers conduct bio-psycho-social assessments, develop individualized care plans with each client, and provide linkages to services to meet each person's identified needs. Those services include access to and maintenance of primary medical care and medications, transportation for HIV care related services, and a variety of additional medical and support services for their clients.
